MOH's One Care Team

As a caregiver, I recently observed something that made me think differently about healthcare.


Different healthcare professionals attended to the same patient, yet the care felt coordinated and seamless.


As a family member, it gave me confidence that everyone seemed to understand the patient's overall care journey.


Looking back, I realised I had already experienced the kind of coordinated care that šŒšŽš‡'š¬ šŽš§šž š‚ššš«šž š“šžššš¦ aims to achieve. At the time, I didn't know the name—I only knew that the care felt reassuring and well coordinated.


It reminded me that healthcare sustainability isn't only about increasing manpower. It's also about designing systems that help patients receive the right care, from the right healthcare professional, at the right time.


Wanting to understand this better, I learned more about the šš«š¢š§šœš¢š©ššš„ šƒšØšœš­šØš« š¦šØššžš„, which highlights the importance of coordinating a patient's care journey across different healthcare professionals.


Better systems don't replace healthcare professionals. They help them work together to deliver more coordinated care.


Thank you to the Ministry of Health (Singapore) for sharing information on the Principal Doctor model, which inspired me to learn more about this important initiative.

This reminds me that building a sustainable healthcare system is not just about manpower. It is also about creating systems, such as One Care Team, that support coordinated, patient-centered care.
